After four years of incremental updates, Apple appears ready to substantially rethink its most rugged smartwatch.

According to multiple reports from DigiTimes, Apple is preparing a “full redesign” of the Apple Watch Ultra for a release expected in autumn 2026. If accurate, it would be the first significant visual overhaul of the Ultra line since its introduction in 2022.

So far, each generation has largely preserved the same industrial design. The Ultra 2 brought a brighter display, while the Ultra 3 added a new black finish. Beneath the surface, however, the device remained fundamentally unchanged in form. The next version may break that pattern.

A shift beyond aesthetics

The reports do not offer concrete details about the new design, but industry observers cited by DigiTimes expect changes that go beyond cosmetic adjustments. Some manufacturing signals suggest early production activity beginning as soon as July, in line with a possible September launch alongside the iPhone 18 lineup.

Analysts also expect the redesign to have commercial impact, with projections pointing to a 20 to 30 percent increase in shipments compared with 2025.

A stronger focus on health monitoring

More significant than the design itself may be Apple’s continued expansion into health sensing.

DigiTimes reports that the Apple Watch Ultra 4 will feature a major upgrade to its sensor system, potentially including a new arrangement of eight sensors placed in a ring on the back of the device. While Apple has not confirmed this configuration, the direction aligns with a broader trend: more sensors, more continuous data, and more advanced health interpretation.

One of the key features under discussion is a new form of high blood pressure notification. The system would use the optical heart rate sensor to analyze how blood vessels respond to each heartbeat, flagging abnormal patterns that may indicate hypertension risk.

Unlike existing hypertension notifications introduced with watchOS 26, which rely on longer-term trend analysis over roughly 30 days, this approach is described as more immediate and potentially more clinically refined. According to the reports, the feature is currently under review by the U.S. Food and Drug Administration.

Apple has steadily expanded Apple Watch’s medical capabilities in recent years, positioning the device between consumer wellness tools and regulated health technology. A more formally validated hypertension system would mark another step in that direction.

Toward a more independent device

Beyond health tracking, the Ultra 4 is also expected to move further away from dependence on the iPhone.

Reports point to improved connectivity, including possible adoption of 5G RedCap, a version of 5G designed for wearable and low-power devices. The goal is to improve connectivity without significantly reducing battery life, one of the key constraints for wrist-worn devices.

Satellite messaging has also been mentioned as a potential feature. While unlikely to be used frequently, it would extend communication capabilities in remote environments where cellular and Wi-Fi coverage are unavailable.

Battery life improvements are also part of the broader expectations, with some reports suggesting up to 48 hours of use, compared with around 42 hours today.

A broader “Ultra” strategy

The Apple Watch is also increasingly seen as part of a wider branding shift. In recent months, Apple has been linked to potential “Ultra” versions of several product lines, including a rumored foldable iPhone and new MacBook models.

Within that context, the Apple Watch Ultra 4 may play a defining role in shaping what the “Ultra” label represents: durability, autonomy, and advanced capability in environments where the iPhone is not always present.

Incremental change, or a turning point

Many of the most ambitious features remain unconfirmed, and Apple’s plans could still change before launch. Touch ID, for example, has been rumored in earlier reports but is now considered unlikely.

Even so, the direction of travel is becoming clearer. If the combination of new sensors, improved health monitoring, expanded connectivity, and modest design changes all materialize, the Apple Watch Ultra 4 would represent the most substantial update to the line since its debut.

Not necessarily a reinvention in appearance, but potentially a meaningful step in function: a device designed less as an accessory to the iPhone, and more as a capable system in its own right.
